Job Description:

Position Summary:

This role is responsible to develop, plan, implement, and evaluate clinical pharmacy programs as assigned. Oversee and coordinate assigned programs and work collaboratively to provide effective patient education and training.

Here’s what you’ll do:

  • Develop, plan, implement, and evaluate clinical pharmacy programs as assigned (i.e., Medical Therapy Management programs; health screening programs; etc.)

  • Oversee and coordinate the Ambulatory Care services provided to contracted groups.

  • Participate in required planning and implementation calls, work as a collaborative member of the designated care team.

  • Achieve goals set forth by SpartanNash and the practices for each assigned program.

  • Oversee and coordinate the Company Direct Employee Wellness Program (CDEW) to include: - Coordinate visits for current CDEW members and prospective clients. - Interview, select and train Pharmacists as necessary. - Participate in and/or supervisor clinical events (meetings with physicians and patients, disease management and related testing, etc.) as needed. - Develop protocols for new disease states (minimum of one per year) and implement new clinical programs for new disease states.

  • Oversee and coordinate the Immunization Program to include: - Obtain and maintain immunization trainer status. - Coordinate, assist with and/or oversee flu clinics and flu vaccination schedules for Company pharmacy locations and ensure accurate staffing and usage of Pharmacists. - Develop complete policies and procedures for immunizations. - Oversee MCIR (Michigan Care Improvement Registry); complete user agreements, coordinate site administrators within the Company, ensure pharmacy locations have accurate forms/statements to distribute to patients, ensure accurate and timely submission of immunization records, etc. - Develop and implement various immunization programs (i.e., shingles, travel immunizations, etc.), work with Pharmacy Operations Team to reach set goals and objectives.

  • Oversee and coordinate the SpartanNash-Ferris State University PGY-1 Community Pharmacy Residency Program to include: - Develop and coordinate residency program and assigned learning experiences.

  • Coordinate and/or facilitate communication with universities and colleges related to Company's program. - Interview and select resident(s) for the Residency Program.

  • Develop effective working relationships and partner with clients, physicians, patients, dieticians, nurses, etc. to provide collaborative care.

  • Maintain current knowledge of legal regulations, industry trends and best Pharmaceutical/Clinical Care practices; make recommendations to improve current programs and processes and to ensure company legal compliance.

  • Additional responsibilities may be assigned as needed.

Here’s what you’ll need:

  • Bachelor's Degree (Required) Pharmacy or related field required.

  • Doctorate Degree (Preferred) Pharmacy

  • Board Certified in Ambulatory Care of equivalent (Preferred)

  • Diabetes Certification (Preferred)

  • Immunization Certification (Required)

  • Point of Care Testing Cert (Preferred)

  • R.Ph License in good standing (Required)

  • Three (3) years Pharmacy/Clinical Care experience preferred.

  • Familiar with Disease State Training and proficient in Soap Notes (subjective, objective, assessment, and plan) method of documentation.

  • Ability to effectively discharge duties related to patient care and work collaboratively with care team and patients.

  • Strong written and verbal communication; ability to develop positive working relationships with both internal and external customers.

  • Strong presentation and training skills.

  • Strong organization, prioritization and time management skills.

  • Strong analytical, decision-making, problem solving project management skills.

  • Proficient in MS Office (Word, Excel and PowerPoint).

  • Ability to travel up to 30% to retail and client locations, minimal overnight travel to conferences.

While performing the duties of this position, the associate is subject to a typical office environment and is rarely exposed to outside weather conditions. Temperatures may vary for those subject to any of the following areas: computer/server room, print shop, production area). The noise level in the work environment is usually low to moderate but may be high in distribution settings. Travel requirements vary by assignment.
